Technical Assistance:
The provider has not completed all 16 hours of required annual training. The provider shall complete her training hours and forward such documentation to the Inspector. The license cannot be processed for renewal until the training documentation is received by the Inspector.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 10 points. Children's records, adult records, caregiver records, caregiver training, background checks, emergency preparedness items, the physical plant, children's nutrition, and programming were inspected today. The children were observed eating morning snack from home, playing with toys, interacting with each caregiver, and engaging in creative play activities. This inspection was completed between approximately 9:45am and 12:15pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-900-A
Description: Based on observations, it was determined that one child under the age of 4 was served a food that was not prepared in a way to assist in preventing choking. Evidence: Upon arrival, the Inspector observed a three year old child eating grapes. These grapes were whole and were not prepared in a manner to reduce the risk of choking.

Plan of Correction: Each child brings their own food from home. I will make sure any possible choking hazard foods are either not served or are prepared in a way to prevent choking.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that two records are not complete. Evidence: Child record B and record C do not include a physical exam report. Each child has been in attendance for more than 30 days.

Plan of Correction: Both parents will be reminded to bring their child's physical form.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The record for assistant caregiver A does not include two completed reference checks.

Plan of Correction: I will document two reference checks.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: 1. Child record B does not include the parent's medical insurance policy number.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of administrative documents, it was determined that three parents have not been provided all required written information. 1. The parents of child A and child B have not signed the provider's Medication Administration policy. 2. The parents of child C have not signed the Provisions of the Emergency Plan document.

Plan of Correction: Each parent will be asked to sign their form.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's health records, it was determined that two records are not complete. Evidence: Child record B and record C do not include an immunization report.

Plan of Correction: I will remind each parent to bring the shot record.
