Discussion was held on the topics of current enrollment, current staffing, new expiration dates for all background checks that have increased to 5 years, and a check for the annual fee of $14 was given to the Inspector to be processed. Discussion was also held of the topic of the newly acquired CDA certificate obtained by the Provider at the end of 2018.
The following inspection took place between the hours of 10:30 am through 12:30 pm. Today, I met with the Provider on site at the Passa Family Day Home (FDH) located in Alexandria, Virginia. At the start of this unannounced monitoring inspection there were a total of 6 children (22 points) with 2 caregivers. A total of 6 children?s files, 3 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: toy play, music, infant sleeping, tummy time, infant feeding, lunch, and other organized activities. Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 6 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child who was not exempt from the immunization requirements according to subsection B of this section once every six months for children under the age of two years. Evidence - 1. On the date of inspection (02/13/2019), current immunization documentation for Child #2 was not available for review. 2. The immunization documentation on file for Child #2 was dated - June 5, 2018. 3. Child #2 was in attendance during today's inspection. Plan of Correction: The Parent of Child #2 shall be contacted and an updated immunization page shall be obtained.
