An unannounced renewal inspection was completed on 2/25/2019. There were three children in care with the licensee. The point total for the three children was six. Five children's records were reviewed, the provider's record was reviewed, and the household member's record was reviewed. No medications at the family day home. The inspector arrived at the provider's home at 10:10 am and departed at 12:30 pm. The children were observed during am snack, free time, during a music and movement activity, story time, and during a diaper change. Discussion with licensee about: renewal application and finger print process.

Standard #: 22VAC40-111-60-B
Description: Based on record review of five children's records, the provider failed to ensure that two children's records have all required information. Evidence: 1. Children # 2 and 4 did not have a documented start date.

Plan of Correction: The provider will document the start date. In the future all children will have all required info documented.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that all chemicals were not accessible to the children in care. Evidence: 1. The cabinet under the sink which contained all the cleaning chemicals in the kitchen was not lock during the inspection and was accessible to the children in care.

Plan of Correction: The cabinet has been locked and will be monitored.
