63.2 Facilities and Programs
An unannounced monitoring inspection was conducted today from 12:03pm-2:05pm. There were 12 children (28 points) supervised by 3 direct care staff. The home, 3 staff records, 8 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed watching a video, playing with dolls, tea sets, cars and trucks, eating lunch and napping. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Standard #: 22VAC40-111-700-A Description: Based on a review of medications, a prescription medication did not have written authorization from the parent. Evidence: Child #3's (start date: 8/3/2015) Epinephrine did not have written authorization from the parent. Plan of Correction: The form was misplaced but found and added to his file.
Standard #: 22VAC40-111-60-B Description: Based on record review, 4 of 8 children's records did not contain all of the required information. Evidence: 1. Child #1's (start date: 3/30/2016) record did not contain documentation of parental acknowledgement of the family day home's medication policy, and annual review of emergency contact information performed by the parent. The most recent documented review of emergency information was dated 7/25/2017. 2. Child #2's (start date: not documented) record did not contain documentation of proof of identification, first day of attendance, and parental acknowledgement of emergency medical care. 3. Child #7's (start date: 10/6/2014) record did not contain documentation of the father's work telephone number. 4. Child #11's (start date: 6/25/2018) record did not contain documentation of the father's work telephone number and medical insurance policy number. Plan of Correction: Records will be updated promptly.
Standard #: 22VAC40-111-200-A Description: Based on record review, 1 of 3 caregiver records did not contain documentation of orientation. Evidence: Caregiver A's (date of hire: 8/2/2016) record did not contain documentation of orientation. Plan of Correction: Her file was updated.
Standard #: 22VAC40-111-220-A Description: Based on observation and record review, the family day home did not ensure that a caregiver has satisfactorily completed a training program for this purpose developed or approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist. Evidence: Child #3 (start date: 8/3/2015) has a diagnosed allergy and emergency medication on site. During the time of inspection, there was no caregiver qualified to administer medication. Plan of Correction: The Substitute provider was enrolled in MAT class.
Standard #: 22VAC40-111-240-A Description: Based on observation, areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition. Evidence: 2 out of 3 infant cribs in the naptime room had chipped and peeling wood along the top edges of the cribs. Plan of Correction: We replaced those two cribs with extras that we had.
Standard #: 22VAC40-111-600-B Description: Based on observation, the family day home did not ensure that infants and toddlers spend no more 30 minutes of consecutive time confined in a crib, play pen, high chair or other confining piece of equipment. Evidence: From 12:03pm to 12:39pm, Child #2 (date of birth: 1/11/2018) was strapped in a high chair. Plan of Correction: We had a retraining regarding this licensing standard.
