63.2 Child abuse and neglect
An unannounced mandated Renewal inspection was conducted today from 10:00am to 11:30 am. There were 7 children (15 points) and 2 caregivers present. Children were observed drawing, playing with dolls, pretending to be dogs, building with blocks, and sitting in the toy trunk to "go to the hair salon". Reviewed 2 staff files, 2 household members files and 3 children's files. Reviewed attendance, first aid and emergency supplies, emergency evacuation log, and physical plant. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination under the direction of a physician within 30 days after the first day of attendance. Evidence: Child#1 (5/17/15) did not have documentation of a physical. Plan of Correction: I will ask the parent for a copy
Standard #: 22VAC40-111-320-G Description: Based on record review, the wood burning fireplace did not have documentation of the annual inspection. Evidence: Interview with Provider revealed she could not find the documentation of the annual inspection/cleaning. Plan of Correction: I will send you a copy of the document
