54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitorin inspection was conducted today from 9:30am-11:30am. There were 11 children (36 points) supervised by 3 direct caregivers. The home, staff records, children?s records, household member?s records, evacuation drills, programming and policies were inspected. There were no medications on site today. Children were observed engaging freely, singing, reading and snack time. Diapering procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Positive interaction between children and caregivers was also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on observation, not all bottles are labeled with child's name and date. Evidence: Bottles located in the refrigerator were all labeled with child's name, but some did not have a date.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-60-B
Description: Based on review of 11 children's records, 2 emergency contacts are not documented in 1 record. Evidence: Child #3 has documentation of 1 emergency contact.

Plan of Correction: I have been in contact with mom, trying to get another one. I will follow up.

Standard #: 22VAC40-111-90-A
Description: Based on review of 11 children's records, not all have documentation of an immunization record before the first day of attendance. Evidence: Child # 4, start date 2/25/19, does not have documentation of an immunization record.

Plan of Correction: I will get a copy of her immunization record.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, not all children under the age of 2, have documentation of an updated immunization record every 6 months. Evidence: Children #1 & #2, who are under the age of 2, do not have documentation of an updated immunization record. Current immunization records on file are 3/2/18 & 8/14/18 respectively.

Plan of Correction: I will get an update record for each child.

Standard #: 22VAC40-111-680-A
Description: Based on observation, caregivers hands were not washed after a diaper change. Evidence: Staff was observered changing a diaper, then took child into play area without washing their hands.

Plan of Correction: We usually do, we will make sure we do it every time.

Standard #: 22VAC40-111-680-B
Description: Based on observation, infants hands were not washed after a diaper change. Evidence: Caregiver did not wash child's hands after a diaper change.

Plan of Correction: We normally do, we will make sure we do it every time.
