Comments:
An announced initial inspection was conducted on March 7, 2019. No children were present and no children are currently enrolled. One room will be used exclusively for childcare. Space and equipment is appropriate for preschool age children. Transportation will not be provided and medication will not be administered. Inspection will be reviewed by Licensing Administrator regarding license issuance.

Standard #: 22VAC40-111-760-A-1
Description: Based on observation, provider did not ensure the first aid kit has required items. Evidence: On March 7, 2019, the first aid kit did not contain triangular bandages.

Plan of Correction: Provider will pick up all items this weekend.

Standard #: 22VAC40-111-790
Description: Based on interview, provider did not ensure emergency numbers to include 911, regional poison control number and person responsible for back up care are posted. Evidence: On March 7, 2019, provider stated she does not have emergency phone numbers posted.

Plan of Correction: All information will be posted by this weekend.

Standard #: 22VAC40-111-940-B
Description: Based on interview, provider did not ensure there is an operable thermometer available to monitor refrigerator and freezer temperatures. Evidence: On March 7, 2019, provider stated she does not have a thermometer in the kitchen refrigerator and freezer.

Plan of Correction: Provider will pick up all items this weekend.

Standard #: 22VAC40-111-240-A
Description: Based on observation, provider did not ensure areas and furnishings of the family day home, inside and outside, are maintained in a clean, safe and operable condition. Evidence: On March 7, 2019, the rear door that will be used for entrance into the family day home was not operable as the door would not open.

Plan of Correction: Provider will have the door looked at and have another lock put on it.

Standard #: 22VAC40-111-320-G
Description: Based on interview, provider did not ensure a wood burning fireplace is inspected annually by a knowledgeable inspector to verify the device is properly installed, maintained and cleaned as needed. Evidence: On March 7, 2019, provider stated the fireplace is wood burning and does not have documentation of an inspection.

Plan of Correction: Provider will have an inspection conducted.

Standard #: 22VAC40-111-330-A
Description: Based on observation, provider did not ensure there is a landline telephone, excluding a cordless or cell phone, available, operable and accessible. Evidence: On March 7, 2019, there was not a landline telephone available that was not cordless.

Plan of Correction: Provider will pick up all items this weekend.

Standard #: 22VAC40-111-690-D
Description: Based on interview, provider did not ensure there is a diapering surface that is absorbent. Evidence: On March 7, 2019, provider stated she does not have a surface designating for changing diapers.
