54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring visit was conducted today between the hours of 9:00am through 10:30am with the provider on site. There were 11 children (24 points) with the provider and two assistants. A sample of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, circle time, reading books, singing, breakfast, bathroom breaks, hand washing, and other organized activities. Interactions between the children and staff were positive. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Areas of non-compliance are identified in the violation notice. Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening. Evidence: 1. Caregiver #1 had a Tuberculosis Screening form dated 11/04/2016 and did not have an updated screening. 2. Household Member #1 had a Tuberculosis Screening form dated 11/11/2016 and did not have an updated screening.

Plan of Correction: I will schedule both of us to have it completed.

Description: Based on observation, furnishings of the family day home inside were not maintained in a safe, and operable condition. Evidence: 1. A red chair had 7 visible cracks in the material.

Plan of Correction: I threw the chair away while the inspector was present.
