Comments:
An unannounced renewal study was conducted today from 11:00am-2:30pm. A total of 10 children [25 points] were present with caregivers. A sample of 6 children's records, 1 household member records, 3 caregiver records, emergency drills, emergency supplies, medications and outdoor play equipment were inspected/reviewed. Children were observed during various play activities, storytime, handwashing, lunch and transitions to naptime. Areas of non-compliance are documented on the violation notice. Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observations and interview, medications were not stored in a locked place. Evidence: Two emergency medications for Child #10 were stored in the children's booksbags in an area not locked and accessible to children.

Plan of Correction: Medications were moved to a locked location during the inspection.

Standard #: 22VAC40-111-800-B
Description: Based on a review of records, annual review of the emergency plan was not documented. Evidence: The most recent review of the emergency plan was documented on 11/3/2017.

Plan of Correction: The plan was reviewed during the inspection and documented; Corrected.

Standard #: 22VAC40-111-120-B
Description: Based on a sample review of records, 1 out of 3 caregiver records did not include the required information. Evidence: The record for Caregiver #2 [date of employment: 3/11/2019] was missing documentation of at least two references prior to employment.

Plan of Correction: I will obtain the references by the end of the week.

Standard #: 22VAC40-111-170-A
Description: Based on a sample review of records, 1 out of 2 caregiver records did not include documentation of a current report of tuberculosis screening prior to coming in contact with children. Evidence: The record for Caregiver #2, who observed caring for children, did not include a report of tuberculosis screening.

Plan of Correction: I will have the caregiver schedule an appointment for the screening to be completed.

Standard #: 22VAC40-111-210-A
Description: Based on a sample review of records and interview, documentation of 16 hours of annual training was not available for 2 out of 3 caregivers. Evidence: 1. Documentation of 3 hours of training was available for Caregiver #1 [February 2018 through February 2019]. 2. There was no documentation of training and development available for Caregiver #3 [February 2018 through February 2019].

Plan of Correction: We are in the process of obtaining additional training hours.

Standard #: 22VAC40-111-320-G
Description: Based on observations and review of documentation, documentation of a annual inspection of the wood burning fireplace was not maintained. Evidence: A wood burning fireplace was observed upstairs in an area separate from the program area. The most recent inspection of the fireplace was conducted 9/20/2013.

Plan of Correction: The fireplace is not being used but I will have an inspection completed or have it permanently sealed.

Standard #: 63.2-1720.1-B-2
Description: Based on a review of records, 1 caregiver did not submit to fingerprinting prior to employment. Evidence: The record for Caregiver #2, who was observed caring for children, did not include a criminal record check via fingerprinting.

Plan of Correction: I will make an appointment for the Caregiver as soon as possible to obtain fingerprints.
