Comments:
An unannounced renewal inspection was conducted today from approximately 4:40pm-5:55pm. There were 4 children (7points) directly supervised by 2 caregivers. A sample of 4 children?s records, 2 caregiver's records and 2 household member's records were reviewed. The children were observed watching television, playing electronics and engaged with caregivers. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-120-B
Description: Based on record review, the assistant's file was incomplete. Evidence: Assistant A was missing the job title and employment date.

Plan of Correction: I will make corrections today for files.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 1 of 4 records reviewed were missing required information. 1) Child A was missing the father's residential information, the father's employer and employer phone number.

Plan of Correction: I will make corrections today for files.

Standard #: 22VAC40-111-600-B
Description: Based on observation a toddler spent more than 30 minutes of consecutive time confined. Evidence: Child B was confined in a baby bouncer from approximately 4:40pm until 5:18pm (43 minutes).

Plan of Correction: I will correct that today. I informed my assistant of this requirement.
