Technical Assistance:
Technical Assistance was provided for the following: Attendance, Adding a gate to the staircase if children under two years of age are enrolled, Background checks.

Comments:
An unannounced renewal inspection was conducted from 10:15am to 12:10pm. There were 5 children (10 points) in care of 2 caregivers. The children were observed playing with toys and play dough, coloring pictures, using the bathroom and washing hands. 5 children's records, 2 caregiver's records and 2 household member's records were reviewed. Emergency drills records were current and a walkthrough of the family day home was conducted. There were no medications on site. Areas of non compliance are listed in the Violation Notice. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a subsequent Tuberculosis (TB) screening every two years, for each household member. Evidence: Household Member # 1's most recent TB screening expired on 2/13/19.

Plan of Correction: I will get updated TB for household member.

Standard #: 22VAC40-111-210-A
Description: Based on record review, a caregiver at the family day home did not complete the annual 16 hours of training. Evidence: Caregiver # 1's records indicated that she only completed 11 hours of childcare related trainings between 1/23/18 and 1/23/19.

Plan of Correction: I will her complete 5 hours within the next two weeks.
