Technical Assistance:
Discussed the new renewal application and process with the Provider.

An unannounced mandated inspection took place between the hours of 10:30am-12:20pm. There were 9 children (25 points) in the direct care and supervision of 3 caregivers. A total of 4 children?s records and 4 caregiver/household member?s records were reviewed. The evacuation drills, emergency plan, first aid kit, and training records were also reviewed. The children were engaged in the following: Playing with toys, eating lunch, diapering, playing outside, singing songs and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 out of 4 children's records did not include a physical examination prior to a child's attendance or wthin 30 days after the first day of attendance. Evidence: 1. Child #3 started on 2/21/2018 and did not have a physical examination on file.

Plan of Correction: I will ask the parents at pick-up today to bring the physical exam in right away.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, 2 out of 4 staff/household member records did not have a current sworn statement. Evidence: 1. The Provider's sworn statement could not be located. 2. The household member's most recent sworn disclosure on file had a date of 2/10/2014.

Plan of Correction: New sworn statements were filled out and signed during the inspection.
