An unannounced monitoring inspection was conducted today from 12:15pm-2:15pm. There were 3 children (7 points) directly supervised by the provider The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, watching educational videos, eating snack, and napping. There was an abundant supply of books, toys, and materials for the children. Standard #: 22VAC40-111-90-C Description: Based on record review, additional immunizations were not obtained once every six months for a child under the age of two years. Evidence: Child#1's record contains an immunization record that was last updated on 6/1/2018, the child was 1 year old the last time their immunization record was updated. Plan of Correction: Today we will request new immunization records from the parents.
Standard #: 22VAC40-111-320-G Description: Based on record review and discussion with the provider, the wood burning fireplace was not inspected annually by a knowledgeable inspector to verify that the device was properly installed, maintained, and cleaned as needed. Evidence: The wood burning fireplace, locate in the programming area, was not inspected annually. Plan of Correction: We will completely and permanently seal the fireplace.
Standard #: 22VAC40-111-430-B Description: Based on observation, the Family Day Home has a pressure mounted gate installed at stair openings. Evidence: a pressure mounted gate was observed at the bottom of the stairs on the main level of the home, no hardware installed gate was in use as a protective barrier at the opening of the stairs. The children were using the main level of the home during the inspection, Child#1 (under the age of 2 years) was also observed using this area of the home. Plan of Correction: We will look for the appropriate gate.
