Technical Assistance:
Reminded provider that the daily menu must be kept up-to-date.

Comments:
An announced Initial Inspection was conducted today from 10:00am to 12:15pm. There were 3 children (8 of points) with the Provider present today, within the supervision guidelines. Provider and 3 household members records were reviewed and found complete. All enrolled children?s files were reviewed. Overall, good systems are in place. The home was found clean and supplied with ample toys and programming equipment appropriate for the children currently in care. Adequate space determined for the requested capacity of (5) children. Children were observed doing puzzles, coloring, singing songs and eating lunch. On the menu for today is (ramen noodles, mixed cooked vegetables, strawberries and milk). The provider is currently MAT trained to administer medications to the children; there is no medication on site at today's inspection. Emergency procedures, provider contract, First Aid/CPR certification was verified. Violations found today were reviewed with the provider and an appropriate corrective action was provided. Standard #: 22VAC40-111-770
Description: Based on observation, the provider does not have a working battery-operated water band radio. Evidence: The weather band radio that the provider has available does not turn on.

Plan of Correction: I have batteries that I'll replace them today.

Standard #: 22VAC40-111-980-A
Description: Based on interview and record review, the provider has not obtained written permission to take the children off the premises for regularly scheduled trips to the playground from the parents of each child. Evidence: The provider stated that for outside play she takes the children to the community playground that is down the street from the family day home. 3 out of 3 children's records did not contain permission to leave the premises.

Plan of Correction: I'll make a permission form today while the children are napping and I'll have the parents sign it when they pick up today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain the address for their second emergency contact or the families medical insurance policy number. Child #2 file did not contain the address for their first emergency contact. Child #3 file did not contain a second emergency contact, the address for their first emergency contact or documentation that the age and identity of the child had been verified by the provider.

Plan of Correction: When the parents pick up tonight I'll give them a copy of their forms and ask them to provide the missing information tomorrow.

Standard #: 22VAC40-111-290
Description: Based on observation, sharp objects were accessible and not closely supervised. Evidence: While the provider was in the kitchen preparing lunch for the children, the children were given construction paper and pens to color. Child #3 was observed walking around the play area with their pen in their mouth. The provider acknowledged Child #3 walking with the pen in her mouth and asked the child if it tasted good. The pen was not removed from the child's mouth.

Plan of Correction: She won't have the pen anymore.
