63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was completed on March 4, 2019 from approximately 2:15p.m. to 4:15p.m. Five children were in care, infants, for a total of twenty points. There is one enrolled school aged child who had not arrived yet. The children were observed interacting with the provider and an assistant during infant activities, hand washing, diapering, and snack time. Children?s learning materials and equipment, the rest room, inside and outside areas and the home?s first aid kit and emergency supplies were inspected. Five children?s records, two caregiver records, fire drill and shelter in place logs, the emergency preparedness and response plan, daily attendance and required postings were reviewed. Transportation is not provided and medication is not administered. There were no injury reports to review. The provider was available for interview, inspection and exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-111-170-A
Description: Based on a review of caregiver records, the licensee failed to obtain a current tuberculosis screening at the time of hire and prior to coming into contact with children for one of two caregivers. Evidence: 1. Caregiver #2's record, hired September 4, 2018, contained a tuberculosis screening dated November 28, 2018 and therefore not within the required time frame. 2. Caregiver #1 acknowledged this was not obtained within the required time frame.

Plan of Correction: The provider will ensure that she obtains documentation of a current tuberculosis screening at hire for any new caregivers.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver records, the licensee failed to ensure caregiver obtained 16 clock hours of annual training for one of two caregivers. Evidence: 1. Caregiver #1's record, hired in January 2010, reflected approximately eight hours of annual training for 2018. 2. Caregiver #1 acknowledged there was not enough hours of annual training completed last year for Caregiver #1.

Plan of Correction: The provider will make sure that training will be set up every two months for all caregivers throughout the entire year.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver records, the licensee failed to obtain a central registry finding within 30 days of employment for one of two caregivers and employment was not discontinued. Evidence: 1. Caregiver #2's record, hired on September 4, 2018 contained a central registry finding dated December 12, 2018 and therefore not within the required time frame. 2. Caregiver #1 acknowledged this was not obtained within the required time frame.

Plan of Correction: The provider will ensure that she obtains the central registry finding with 30 days in the future for any new hires. The provider will follow up with the Office of Background Investigations on any checks not received within 30 days and documented what is reported.

Standard #: 63.2-1721.1-B-2
Description: Based on review of caregiver records, the licensee failed to obtain fingerprint results for one of one agents hired prior to January 22, 2018, by the required date of September 30, 2018. Evidence: 1. Caregiver #1's record, hired in January 2010, contained fingerprint results dated October 2, 2019 and therefore not within the required time frame. 2. Caregiver #1 acknowledged fingerprint results were not obtained by September 30, 2018.

Plan of Correction: The provider will not hire anyone new without obtaining fingerprint results first.
