The licensing inspector conducted an unannounced monitoring inspection on 03/18/2019 from 9:25am to 11:17am. The children were observed playing with toys and puzzles. Today, there were 5 children, totaling 13 points, present with three caregivers. The caregivers were observed interacting with the children and cleaning. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children. The provider does administer medication. The provider does not provide transportation. The provider does have a pet. Five children?s records, three caregiver records, and two household member records were reviewed during the inspection. Last emergency drill: 03/12/2019 Last shelter-in-place drill: 03/12/2019 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-90-A Description: Based on review, the provider did not ensure that one of five children's records contained documentation that the child has been adequately immunized before the child attends the family day home. Evidence: The record of child #4 (start date: 8/16/17) contained documentation of immunizations dated 10/2/2018. Plan of Correction: In the future I will get and maintain documentation upon enrollment.
Standard #: 22VAC40-111-250-A Description: Based on observation, the provider did not ensure that hanging items including, but not limited to, window blind or curtain cords, appliance cords, and ropes were out of reach of children under five years of age. Evidence: There were two ropes about 7 feet long hanging from a small tree in the backyard within the reach of children, presenting an entanglement hazard. Plan of Correction: I will remove ropes by 3/18/19.
Standard #: 22VAC40-111-320-B Description: Based on observation, the provider did not ensure that child-resistant protective covers larger than 1-1/4 inches in diameter were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: There was one uncovered outlet in the bathroom that the children use, within the reach of children. There were two uncovered outlets in the kitchen, out of the reach of children. Plan of Correction: I will cover plugs by 3/18/19.
