An unannounced monitoring inspection was conducted on March 13 2019 between 11:00am and 1:45pm. There were 3 children in the direct care of the provider. Children were observed washing hands, playing with toys, and preparing for lunch. The day care space has plenty of materials, including books, toys, and art supplies, for children to utilize. Standard #: 22VAC40-111-800-A
Description: Based on observation, the family day home did not have documentation of an annual review of the emergency plan. Evidence: The emergency plan was documented as reviewed on May 2017.

Plan of Correction: I will review that today.

Standard #: 22VAC40-111-830-A
Description: Based on observation, the provider did not document all monthly evacuations. Evidence: Evacuations were not documented as conducted on August 2018 and February 2019. Shelter-in-place drills were conducted during those months instead.

Plan of Correction: From today's date I will make sure to get my drills documented.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 3 children in care did not have complete records. Evidence: Child # 2 and Child #3 only have proof of identification and records of current physicals and immunization records. The child records do not contain documentation of the information for parents, medication administration form, and general children's information.

Plan of Correction: I will remind the parents to bring the information tomorrow.
