Technical Assistance:
**Discussed with the provdier that her fire extinguisher needed to be removed from the box so that she has quick and easy access to it in the event of a fire, or other emergency.

Comments:
An Unannounced Monitoring Inspection was conducted today from 1:09pm to 2:24pm. There were 10 children (25 points) directly supervised by 2 staff. The home, 2 staff records, 10 child's records, household member's records (husband), evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely in the play room and napping in the nap room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-90-C
Description: Based on review, the provider did not obtain updated immunization records for children under 2 years of age, every 6 months. Reviewed 10 children's records and found that 1 (Child 1) did not have documentation of an immunization record within the last 6 months. Last documented record was from May 2018.

Plan of Correction: Will text the mom and ask for it today.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider's diapering surface was not non-absorbent, as it had several small tears in the surface exposing the soft cushion underneath.

Plan of Correction: Will purchase a new diapering pad today.
