The licensing inspectors conducted an announced focused monitoring inspection on Monday, March 25, 2019 from approximately 9:38 am until 1:15 pm. During the inspection, there were observation of ten children in care totaling 25 points with two caregiver present. The licensing inspector reviewed the designated children care areas, bathrooms, kitchen area, outdoor play areas, and emergency supplies at the family day home to determine compliance with licensing standards. During the inspection, the licensing inspectors completed observations of the children participating in various activities to include interacting with peers and caregivers, playing outdoor with equipment, completing bathroom breaks, diapering by caregiver, transitioning indoors, eating lunch and preparing for napping. There were five children's, one caregiver's, two assistant caregiver's/volunteer's, and one household member's records reviewed during this inspection. Standard #: 22VAC40-111-890 Description: Based on observations, the caregiver did not ensure proper labeling for meals and snacks brought from child's home. Evidence: During the inspection on March 25, 2019, three out of four lunch bags observed in the childcare area without being clearly labeled of the owner. Plan of Correction: The parents will be informed to label all food and cups brought from child's home in addition to individual bags.
Standard #: 22VAC40-111-900-A Description: Based on observation, the caregiver did not ensure to assist in preventing choking. Evidence: During the inspection on March 25, 2019, three out of the five children were observed at the lunch table given an uncut half piece of banana by caregiver which was deemed thick and slippery and a choking hazard due to children being under four years of age. Plan of Correction: The caregiver will ensure all food items will be cut to prevent choking.
Standard #: 22VAC40-111-240-A Description: Based on observations, the caregiver did not ensure all areas were maintained in a clean, safe, and operable condition. Evidence: During the inspection on March 25, 2019, there were two pieces of outdoor equipment to include a red wagon and a green chair with multiple areas of visible rust in the playground area. Plan of Correction: The caregiver will removed from children access until they can be painted or reblasted.
Standard #: 22VAC40-111-280 Description: Based on observations at the family day home, the caregiver did not ensure all poisonous substances were stored in areas inaccessible to children. Evidence: During the inspection on March 25, 2019, the following physical plant observations were completed: 1. There were two cans of paint and one bottle of bleach located on the floor by the exit door of the outdoor play area. 2. There were multiple cans of paint, primer, sealer, car cleaners, and tubes of silicone located on a shelf inside the garage that was unlocked and accessible the the children in care. 3. There was one can of paint located nearby the playground area inaccessible to the children. Plan of Correction: 1. The paint cans will be removed in placed in the loft. 2. The caregiver will ensure the garage door is locked until in use. The caregiver will remove all hazardous substances. 3. The paint cans will be removed and taken to the trash tomorrow
Standard #: 22VAC40-111-680-A Description: Based on observations, the caregiver did not ensure to wash their hands with liquid soap and warm running water Evidence: During the inspection on March 25, 2019, there were two observations of the caregiver after diapering touching the safety gate between the kitchen the designated child care area without washing their hands with liquid soap and warm running water. Plan of Correction: The caregiver will ensure the safety gate is unlatched to get through without touching prior to handwashing with liquid soap and warm running water.
Standard #: 22VAC40-111-680-B Description: Based on observation, the caregiver did not ensure all children's hands were washed with liquid soap and warm running water after diapering. Evidence: During the inspection March 25, 2019, the following observations were completed: 1. There was observation at the family day home of Child#1 and Child#2 hands not being washed with liquid soap and warm running water after diapering. 2. The nine out of ten children's hands were not washed with liquid soap and warm running water before eating. Plan of Correction: The caregiver will purchase a secured safety step so children won't fall while washing hands due to their ages. The caregiver will use this method instead of soapy rag.
