A renewal study was conducted with the provider, who was present with four napping children for a total of 10 points. For lunch, the provider reported that the children were served spare ribs, mash potatoes, green beans or corn, strawberries and milk. There were no medications on site and there have been no injuries/accidents. The provider's first aid and CPR are through American Red Cross and is due to expire on 7/30/19. Eight children and two household files were reviewed today. Arrival time: 1:45 pm Departure time: 3: 15 pm
Standard #: 22VAC40-111-100-A Description: Based on a review of 8 children's files and an interview with the provider, it was determined that four children did not have physicals on file within 30 days of enrollment. Evidence: Children A and B enrolled on 7/2/18, child C enrolled on 2/18/19 and Child D enrolled on 11/5/18 and none of these children had physicals on file. Plan of Correction: I thought I had the physicals but it was the immunization records. I will get the physicals from the parents.
Standard #: 22VAC40-111-210-A Description: Based on a review of the assistant's file and an interview with the provider, it was determined that the assistant did not have 16 annual training hours. Evidence: For 2018, Assistant A had 10 of the required 16 training hours. Plan of Correction: My assistant will get the additional training hours.
