Comments:
An unannounced monitoring inspection was conducted on 3/28/19 from 11:15am until 12:45pm. At the time of the tour, there were 7 day care children, plus 1 household child, for a total of 14 points with the provider. A sample of 5 children's records and 2 provider/household member records were reviewed. Children were observed participating in morning learning activities and eating lunch. Hand washing and toileting procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and family day home postings were reviewed. One violation in the area of physical environment and equipment is identified on the violation notice and was discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: There is an electrical outlet on the wall that does not have protective covers and a power strip with 3 uncovered outlets that do not have protective covers.

Plan of Correction: The provider will ensure the outlets have child-protective covers when not in use.
