Comments:
An unannounced renewal inspection was conducted today 3/28/2019 There were eleven children present with two caregivers and total of twenty-seven points. The inspection began at 8:25 am and concluded at 12:20pm. During the inspection diapering was observed, children's center time was observed, morning snack observed, five children's records were reviewed, the provider's records was reviewed, the assistant's record was viewed, emergency supplies were viewed, and some policies and procedures were reviewed. Your renewal application has not been received in the licensing office to date.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on the inspector's review of the provider's emergency plan, the provider failed to ensure an annual review was documented in writing. Evidence: 1. A review of the family day home's emergency plan has not been documented for more than a year. The most recent update was 3/15/2018.

Plan of Correction: The licensee updated the plan and will make sure that the plan is updated annually.

Standard #: 22VAC40-111-60-A
Description: Based on review of five children's records, the licensee failed to ensure that all records were complete. Evidence: 1. Child # 2 did not have the chronic physical problem was completed on the enrollment form.

Plan of Correction: The licensee will have the parent to complete the missing portion of the enrollment form.

Standard #: 22VAC40-111-280
Description: Based on observation, the licensee failed to ensure that all poisonous materials were not stored with food. Evidence: 1. Observed in the bathroom, the cleaning chemicals were stored with the food.

Plan of Correction: The licensee removed the chemicals from the counter where the food was stored and will make sure that they are not stored together .
