Technical Assistance:
Discussed requirement for all S-hooks to remain in the closed position.

An unannounced renewal inspection was conducted on 02/29/2019 from 11:05am - 12:30pm. There were six children in care with two caregivers. The ages of the children counted for a total of 15 points. Children were observed playing games, singing songs and reading books with the assistant in the child care room. Lunch and nap was also observed. Records reviewed for the six children in care as well as the two caregivers. One violation was observed and is listed on the violation notice.

Description: Based on observation, the provider failed to ensure that openings above the ground that are closed on all sides shall be smaller than 3-1/2 inches or larger than nine inches to prevent head entrapment hazards. Evidence: 1. One area of the stairway handrail for the outdoor play structure has six areas that have openings measuring between four and five inches.

Plan of Correction: Repair to this area of the outdoor climber will be made this weekend.
