An unannounced, mandated inspection was conducted today. The inspector arrived at 9:25 AM and departed at 10:45 AM. Two children, totaling six points, were in care with the substitute caregiver. Morning activities and snack were observed. Two caregiver records and seven children's records were reviewed. No children injuries were reported and it was reported that no medication has been administered to children since the previous inspection. An exit meeting was conducted with the substitute caregiver, caregiver 2, prior to closure of the inspection.

Standard #: 22VAC40-111-920
Description: Based upon observation and caregiver interview, the provider has not ensured that a menu for the current one-week period is posted. Evidence: 1. The most recent menu posted on the parent information board is dated for the week of 3/18/2019. 2. Caregiver 2 verified that the menu for the current week is not posted.

Plan of Correction: A current one-week menu will be posted each week.

Standard #: 22VAC40-111-100-A
Description: Based upon review of seven children's records and caregiver interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attending or within 30 days after attending. Evidence: 1. The record provided for child 1 indicated that child 1 began attending on 2/11/2019. The record did not include documentation of a physical examination. 2. Caregiver 2 verified that the record for child 1 did not include documentation of a physical examination.

Plan of Correction: The parent will be required to provide documentation of a physical examination in order for the child to continue attending.
