Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:10am - 12:00pm. There were six children in care for a total of thirteen points with one caregiver. The children were observed playing with the available toys and at story time. Two caregiver's records and five children's records were reviewed. Description: Based on record reviews, the family day home failed to obtain signed documentation for all required information for parents. Evidence: 1. The inspector reviewed five children's records. One of five records review (child record #1) did not contain a signed information for parents document and did not contain the signed provisions of the home's emergency preparedness plan. One of five records reviewed (child record #2) did not contain the signed liability insurance declaration document.

Plan of Correction: The provider will request these documents from the children's parents. The provider will review the children's records to ensure they include all required documentation.
