63.2 Licensure and Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on 04/02/2019 from 11:00am - 12:20pm. At the time of entrance there were four children in care with the provider. The ages of the children counted for a total of nine points. The provider engaged the children with songs, movement activities and reading. Lunch was also observed. Records reviewed for the four children in care, the provider and one household member. Violations were observed in two parts of the standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that children's records contained documentation of an annual review of emergency contact information to ensure the information is correct. Evidence: 1. The enrollment record for child 1 was last reviewed and updated by the parent on 03/12/2017. 2. The enrollment record for child 2 was last reviewed and updated by the parent on 12/07/2017.

Plan of Correction: Each parent will be asked to review and update their child's record today at pick-up.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider failed to provide written information regarding whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. Evidence: There was no documentation of notification of liability insurance on file for child 3 and child 4.

Plan of Correction: Written information regarding liability insurance for the family day home will be provided to each parent today at pick-up.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that furnishings of the family day home shall be maintained in a safe condition. Evidence: 1. Three storage units were not secured to the wall resulting in a toppling hazard. a. These storage shelves/units measured approximately 40 inches in height and 24 inches in width. The two units being used for the parent sign-in area were wobbly when pushed on. b. These units were located in the front of the child care room and were accessible to the children in care.

Plan of Correction: The storage shelves will be secured to the wall or moved by Monday, April 8th.
