Technical Assistance:
Emergency Medication Administration Training (EMAT) was discussed with the provider. Information regarding the training classes can be found at: If the provider decides to begin administering emergency medication, new medication administration - decision to administer medication forms will need to be completed by all parents.

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:15 AM and departed at 12:30 PM. Five children, totaling eleven points were in care with the provider. Morning learning and gross motor activities were observed. Two caregiver/household records and five children's records were reviewed. No children's injuries were reported and no medication was administered to children in care. An exit meeting was conducted with the provider prior to closure of the inspection.

Description: Based upon review of five children's records, the provider has not ensured that each record includes documentation of the parent's required annual review of the child's emergency contact information as required. Evidence; The record provided for child 5 did not have documentation of an annual review of the child's emergency contact information since 11/15/17.

Plan of Correction: The provider responded with the following: All parents will be required to document the annual review of their child's record.
