An unannounced monitoring inspection was conducted today between the hours of 8:45 am through 9:45. There were a total of 3 children, including the provider's own child ( 10 points) with 2 caregivers. A total of 3 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play and blocks and. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-100-A Description: Based on a review of children records, the provider has not obtained documentation of a physical examination within 30 days after the first day of attendance. Evidence: Child #1, first day of attendance 2/11/19 does not have documentation of a physical examination. Plan of Correction: I will ask the parents for the physical exam.
