Technical Assistance:
Discussed the definition of sanitized, using a solution that is sprayed onto the surface of the changing pad.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:00 am through 10:30. There were a total of 6 children (17 points) with 2 caregivers. A total of 4 children's files and no change to staff/household member's files were reviewed. The children were engaged in the following: toy play, writing, breakfast, diapering and hand washing. Violations:
Standard #: 22VAC40-111-960-B
Description: Based on observation of children, Infants who cannot hold their own bottles shall be picked up and held for bottle feeding. Bottles shall not be propped. Evidence: Upon the inspectors arrival an infant was observed in an infant seat. The bottle was propped using blankets. The infant is 9 months and can't hold it's own bottle.

Plan of Correction: We will hold the bottle for the child.

Standard #: 22VAC40-111-960-E
Description: Based on observation and provider interview, bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care. Evidence: There are 5 children with bottles, none of the bottles were labeled with the child's full names or dated.

Plan of Correction: We will label and date all bottles each day.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children records, the provider has not obtained documentation of physical examination within 30 days after the first day of attendance. Evidence: Child #1, date of attendance 2/28/19 and child #2, date of attendance 10/22/18 does not have a copy of the physical examination.

Plan of Correction: I will ask the parents for the physicals.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children records, all records are not complete with the required information. Evidence: Child #1 record does not have the required medical insurance policy number and Information for Parents form.

Plan of Correction: I will ask the parents for these forms.

Standard #: 22VAC40-111-250-C
Description: Based on observation and provider interview, toys hung from the sides with strings or cords have not been removed when the child begins to push up on hands and knees or is five months of age. Evidence: A crib was observed with a mobile attached to the side and hanging over the crib. The Provider stated that child #3 sleeps in the crib. Child #3 record was reviewed, the child is 23 months.

Plan of Correction: I will remove the toy mobile.
