Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, April 2, 2019 from approximately 9:20 am to 11:30 am to determine the provider's compliance with licensing standards at the family day home. During this inspection, there were no children in care, however, the caregiver reported having two children enrolled that attend in the afternoon. The family day home's first aid kit and emergency supplies, posted emergency contacts, fire, shelter-in-place, lockdown drills were reviewed by the inspector. During this inspection, there were two children's records, one provider's record, and two household member's records were reviewed by the inspector. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on April 30, 2019 or the Department cannot renew the license. Standard #: 22VAC40-111-170-A
Description: Based on review of record and interview with caregiver, the provider did not ensure each adult household member has tuberculosis screening as required. Evidence: During the inspection on April 2, 2019, household member #2 did not have a tuberculosis screening available for review. Interview with caregiver revealed a tuberculosis screening had not been completed.

Plan of Correction: The provider will schedule appointment for household member #2 this week.

Standard #: 22VAC40-111-180-A
Description: Based on review of records and interview with caregiver, the provider did not ensure all caregiver and household members has a current tuberculosis screening as required. Evidence: During the inspection on April 2, 2019, the following was reviewed in records: 1. The caregiver's last dated TB was March 13, 2017. 2. The household member #1 last date TB was March 7, 2017. 3. Interview with caregiver confirmed the TB screenings have not be updated.

Plan of Correction: The provider will schedule appointment for TB screening this week.

Standard #: 63.2-1720.1-B-2
Description: Based on review of record and interview with caregiver, the provider did not ensure to submit fingerprinting for all household members as required. Evidence: During the inspection on April 2, 2019, there was not a OBI fingerprinting determination letter found for household member #2. Interview with provider verify OBI fingerprinting has not being completed for household member #2.

Plan of Correction: The provider will complete scheduling appointment for household member#2 tomorrow. The results will be include in record once received.
