Comments:
An unannounced monitoring inspection was conducted today with the two substitute providers, who were present with 9 day care children for a total of 24 points. The children were napping during the inspection. The provider reported that there were no medication on site. Five children and two substitute provider's files were reviewed today. Please provider documentation that you have sent off for the central registry request for substitute provider A by 4/19/19. Reviewed consequences of allowing substitute provider A to return to the day care with out first obtaining the central registry finding. Please submit your plan of correction by 4/12/19. Arrival time: 1:55 pm Departure time: 3: 20 pm

Description: Based on interview with substitute provider B and a review of the attendance log, it was determined that the family day home is not keeping a written record of children in attendance each day. Evidence: Interviews revealed that at no time last week or this week have the substitute providers maintained a written attendance log.

Description: Based on a review of five children's files and an interview with substitute provider B, it was determined that two children did not have files on site. Evidence: Child A and B did not have files on site.

Description: Based on observation and interviews, it was determined that one substitute provider could not speak English in order to perform her job responsibilities. Evidence: The Licensing Inspector (LI) asked a series of questions to substitute provider A such as the number of children present, the ages of some of the children, the names of some of the children, if any of the children had medication, if they maintained an attendance log and the substitute provider was unable to communicate with the LI and looked toward substitute provider B would ask substitute provider A the LI's questions in another language and then substitute provider B would relay the answer in English.

Description: Based on interviews and a review of two substitute provider's files, it was determined that orientation had not been done by the end of the first week of assuming job responsibilities. Evidence: Substitute provider A was hired on 9/24/18 and does not have orientation training.
