Technical Assistance:
Consultation was made regarding playground maintenance.

Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00am through 12:15pm. There were a total of 7 children present (11 points) with the Provider. A total of 5 children?s files, the provider's file, and 2 household member?s files were reviewed. The children were engaged in the following: taking turns doing show and share, learning about sharing, reading books, practicing the Pledge of Allegiance, practicing sight words, playing outside on the playground, and preparing for nap. Positive interactions between the provider and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, the playground, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an abundant supply of age appropriate materials and toys. Description: Based on record review, 1 of 5 children's records reviewed did not contain documentation of an annual review of of emergency contacts. Evidence: 1. Child #5's record did not contain documentation that the emergency contact information was reviewed by the parent within the last year. The record was last reviewed on 1/20/2018.

Plan of Correction: I have spoken to the child's mother who confirmed everything in the file is updated and I will have her sign the emergency contact information review this afternoon.
