Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and upcoming schedule.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Broadlands, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (including 1 resident child) (21 total points) with 2 caregivers. A total of 10 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: outside play, large motor skills on playground, bathroom break, hand washing, language skills, toy play, getting ready for lunch, and other organized activities. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 2 staff files and 1 of 3 household member's files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 22 VAC 40-111-170), every two years. Evidence - 1. On the date of inspection (04/09/2019), the most current TB documentation on file for Staff #2 was dated - 12/09/2016. 2. The most current TB documentation on file for Household Member #1 was dated - 03/21/2017.

Plan of Correction: New documentation has been scheduled to be obtained by next week for all applicable staff and household member.
