Comments:
A Subsidy Health and Safety inspection was conducted on 4/17/2019 to review supplemental health and safety requirements. A total of 5 children were present with 2 caregivers. The children were engaged in the following: Playing with play dough, reading books, listening to music, and eating lunch. Programming, menus, emergency procedures, and 4 children?s records and 2 staff records were reviewed. The provider advised no medications are on site today. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review and staff interview, the daily attendance record does not document the arrival and departure of each child in care as it occurs. Evidence: 1. Child #4 is present today. Child #4 does not have a documented arrival and departure time on Monday, 4/15/19, Tuesday, 4/16/19 or an arrival time on Wednesday 4/17/19. 2. The provider stated that Child #4 was present at the family day home on 4/15/19 and 4/16/19.

Plan of Correction: I will have the parent look at the receipt to write down arrival and departure times.
