Comments:
An unannounced monitoring inspection was conducted today between 12:30pm and 2:30 pm. There were 11 children (21 points) with two caregivers. Household/staff/children's records, emergency supplies, emergency procedures, menu's and attendance records were reviewed. The children were observed eating lunch, having lunch conversations and preparing for nap. Lunch consisted of pizza, carrots, fruit, water and milk. Standard #: 22VAC40-111-200-A
Description: Based on record review, the provider failed to ensure that the assistant would have orientation by the end of their first week of assuming job responsibilities. Evidence: Assistant#2 does not have documentation of orientation by the end of the first week of assuming job responsibilities in the employee file.

Plan of Correction: Provider stated that she completed the orientation, but can not locate form. If I can not locate the form, I will document the training again and place in employee file.
