Technical Assistance:
Licensing inspector discussed changing table, required posting, hazardous substances, pacifiers, sworn statements and child abuse and neglect training with provider

Comments:
An unannounced renewal inspection was conducted today from approximately 12:10pm-2:50pm. There were 8 children ( 25 points) directly supervised by 2 caregivers. A sample of 4 children?s, 2 caregiver's records and 1 household member's records were reviewed. The children were observed during nap time, diaper changing, eating, and during departure. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit missing required contents. Evidence: The first aid kit was missing adhesive tape.

Plan of Correction: I will get it today.

Standard #: 22VAC40-111-830-B
Description: Based on record review, shelter in place procedures were not practiced a minimum of twice per year. Evidence: Shelter in place procedures were not practiced in 2018.

Plan of Correction: I completed one in march.

Standard #: 22VAC40-111-960-E
Description: Based on observation, refrigerated bottles were not labeled with the child's full name and date. Evidence: 1) There are 3 infants in care. 2) The bottle of Child C was not labeled with a full name and date.

Plan of Correction: I will put on. I usually put it everyday.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were incomplete. Evidence: 3 of 4 records reviewed were missing required information. 1) Child A was missing the employer phone number of both parents. 2) Child B was missing the medical insurance policy number. 3) Child C was missing the medical insurance and policy number.

Plan of Correction: Parents will fix the files today.

Description: Based on record review, the assistant did not complete at least 16 hours of annual training. Evidence: 1) The provider stated that they are on an annual calendar of January-December. 2) Assistant A completed 12 hours of annual training for 2018.

Description: Based on observation, the most recent inspection of the facility was not posted. Evidence: The inspection was posted conducted March 2018. The most recent inspection was conducted December 2018.
