Arrived 2:15pm Departed 4:00 pm A Subsidy Health and Safety inspection was conducted on 4/18/2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and is noted on the violation notice. Thank you for your cooperation. Description: Based on a review of three household members files, the facility failed to ensure that all adults have TB test or screening before coming into contact with children. 1. There are no TB test or screening results on file for household members #3 and 4.

Plan of Correction: The provider will ensure that the household members complete a TB test.
