Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 23, 2019. The on-site inspection was from 11:15a.m. to 2:00p.m. The provider and one assistant were caring for 10 children (25 points). There were a total of 13 children enrolled in the program. There were five children's records reviewed along with the records of the provider and four assistants. As is the policy of the provider, there were no medications currently being administered. Two caregivers and two children were interviewed. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Lunch was observed to meet USDA guidelines and consisted of corn dogs, corn, mixed fruit, and milk. Some of the items reviewed and observed included: warm water temperature, required posted information, documentation of evacuation, shelter-in-place, and lockdown drills, refrigerator and freezer temperature, the updated emergency preparedness plan, choice time, TV time, handwashing and bathroom procedures, lunch, nap time, and arrival procedures. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of five staff records and verification with the provider, one assistant failed to obtain a tuberculosis (TB) screening within two years of the most recent screening. Evidence: 1. Staff #3 had not obtained a TB screening within two years of her last screening. Date of her last screening was 03/23/2017.

Plan of Correction: The staff member was able to get an updated (TB) screening during the inspection. The provider will ensure that every caregiver gets a TB screening every two years.
