Comments:
An unannounced mandated inspection was conducted on 04/25/2019 from 9:50 A.M.-12:30 P.M. There were twelve children in care with the provider and two assistants for a total of twenty one points. The records for five children, one provider, one assistant, evacuation drills, first aid kit and supplies, emergency plan, chimney inspection, outside play area and required posted information were reviewed. Children were observed during snack time, handwashing, transition to outside play, outside play, water color painting activities for Earth Day, playdough, free choice, lunchtime, and departure. Standard #: 22VAC40-111-780-B
Description: Based on review of five children's records, the provider failed to obtain a parent's written acknowledgement of review of emergency information annually. Evidence: 1. Review of Child #1 record had no written documentation of a review of emergency information with the parent. The last date reviewed was 04/10/2018. 2. Review of Child #3 record had no written documentation of a review of emergency information with the parent. The last recorded date of review was 07/18/2017. 3. Staff verified the dates of the last annual review of emergency information.

Plan of Correction: The Provider will speak to the parents at pick-up time and ask them to review the information and note any changes needed. Provider will do an audit of all files to ensure that annual updates are completed in a timely manner.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency plan, the provider failed to review the emergency plan at least annually and update the plan as needed. Evidence: 1. The last documented date of an annual review of the emergency plan was April 2016. 2. Provider verified the date of the last annual review of the emergency plan.

Plan of Correction: The provider will review the emergency plan and update accordingly, and place on her calender to review annually.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children records, the provider failed to obtain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Evidence: 1. Child #2's file did not contain documentation of written authorization for emergency medical care. Date of enrollment of Child #2 was 09/04/2018. 2. Staff verified that the file did not contain documentation of written authorization for emergency medical care.

Plan of Correction: The provider will contact the parent at pick-up to obtain the written authorization for medical care.
