Discussed: no equipment, materials or furnishings shall be used if recalled by the U.S. Consumer Product Safety Commission.

Comments:
An unannounced renewal inspection was conducted from 12:00 - 1:20 pm with the provider. There were no children in care today as the provider was closed. Childcare areas and supplies were observed. Required information was posted. Three children's records were reviewed. One provider and one household member record were reviewed and in compliance. The first aid kit, weather band radio and battery operated flashlight were observed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard #: 22VAC40-111-60-B
Description: Based on a review of three children's files, it was determined that not all files were complete. Evidence: the file for child #3 did not have the name, complete address and phone number for two emergency contacts, the medical insurance policy number, or written authorization for emergency medical care.

Plan of Correction: Will ask the parent to provide the missing information and signature.
