Technical Assistance:
Licensing inspector discussed child abuse and neglect training with provider.

Comments:
An unannounced monitoring inspection was conducted today from approximately 3pm-4:30pm. There were 3 children (9 points) in the direct care and supervision of 2caregivers. A sample of 3 children?s records, 2 caregiver's records and 2 household member's records were reviewed. The children were observed napping. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not annually review with the parent the emergency contact information and receive the parent's signed acknowledgment of review. Evidence: Child B's file was last reviewed and signed 02/20/2018.

Plan of Correction: I will fix files today.

Standard #: 22VAC40-111-830-C
Description: Based on record review and interview, documentation of evacuation drills were not maintained. Evidence 1. Evacuation drills were not documented for February/ 2019. 2. Provider stated she forgot to write it down.7

Plan of Correction: I must have forgotten to write it. Other months have been documented.

Standard #: 22VAC40-111-940-B
Description: Based on observation, the provider did not have an operable thermometer available to monitor the freezer compartment temperature. Evidence: There was no thermometer in the freezer compartment of the refrigerator located in the main child care area.

Plan of Correction: I will get one.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's files were incomplete. Evidence: 2 of 3 children's records reviewed were missing required information: 1. Child A was missing 2 of 2 required emergency contacts, first day of enrollment and the employer and employer phone number of both parents. 2. Child B was missing the mother's employer phone number.

Plan of Correction: I will fix files today.
