An Unannounced Monitoring Inspection was conducted today. There were 9 children, 9 points (3 children were picked up by a school bus upon arrival) directly supervised by 1 staff. The home, 1 staff record, 11 child's records, 1 household member's records and a household pet vaccination record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely inside the play room, watching videos on a tablet and playing go-fish with the provider. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-60-B Description: Based on review, the provider did not maintain children's records with the required information. Evidence: Reviewed 11 children's records and found the following: 1) CH4 did not have documentation of a parent's work telephone number 2) CH1 and CH5 did not have documentation of parent verification of annually updated and reviewed children's records. Plan of Correction: Will obtain missing documentation from parents.
