Comments:
An unannounced monitoring inspection was conducted today, April 24, 2019 to determine the providers compliance with family day home standards. The inspector initiated the inspection at 8:55 am and concluded the inspection at 11:05 am. Upon the inspectors arrival there were ten family day home children in care with one caregiver for a total of 26 point. Later an additional caregiver arrived. Children were observed playing with toys and looking at books at the table. Later children did the chicken dance to a music tape. Diapering and bathroom breaks were observed. The caregivers interacted with the children in a positive manner. All areas used for child care, inside and outside, were inspected. The family day home has age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The family day home had the required information posted. The inspectors interacted with the caregivers and children in care. Ten children's records, two caregiver records, and one household member's record were reviewed today. Caregivers have current First aid and CPR. Medications are not administered nor does the provider offer transportation. The most recent evacuation drill was documented on 04/10/2019. The two required shelter-in-place drill were completed on 02/27/2019 and 03/19/2019. A lock down drill was documented on 02/27/2019. Today's lunch consisted of peanut butter and jelly sandwich, mashed potatoes, green beans and milk. The violations cited during the licensing period were checked for corrections. Standard #: 22VAC40-111-780-B
Description: Based on review of children's records, the provider did not annually obtain the parent's signed acknowledgement confirming the review of emergency contact information. Evidence: 1. The record of child #9 was last reviewed by the parent and acknowledged on 02/01/2018, exceeding the requirement to review annually. 2. The record of child #10 was last reviewed by the parent and acknowledged on 08/30/2017, exceeding the requirement to review annually.

Plan of Correction: Per the provider: I will have the parent review the emergency information and sign it tonight at pick up.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The record for Child #8 (22 months in age) with an enrollment date of 10/23/2017, contained an immunization record dated 10/23/2017. Immunization records are required to be updated at least once every six months for children under two years of age.

Plan of Correction: Per the provider: I will ask the parent to bring an updated immunization record.

Standard #: 22VAC40-111-180-A
Description: Based on review of caregiver records, the provider did not obtain documentation of a current tuberculosis screening for each caregiver and adult household member every two years from the date of the first screening. Evidence: 1. The most recent tuberculosis screening for caregiver #1, was dated 04/13/2017, exceeding the two year allowance to renew. 2. The most recent tuberculosis screening for household member #1, was dated 01/10/2017, exceeding the two year allowance to renew. 3. The provider confirmed the two tuberculosis screenings had expired.

Plan of Correction: Per the provider: I will ask my husband to get a new tuberculosis screening today or tomorrow while he is at work. I will update my tuberculosis screening by the end of the week.

Standard #: 22VAC40-111-210-A
Description: Based on review of caregiver records, the provider did not ensure caregivers obtained a minimum 16 hours of annual training in areas areas relevant to their job responsibilities. Evidence: Caregiver #2 with a hire date of 09/10/2016, had documentation of 3 hours of annual training between the period of 09/10/2017 - 09/09/2018 when 16 hours was required.

Plan of Correction: Per the provider: We are signed up to attend some training classes and will complete some on line to get 16 hours.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the licensee did not ensure that a caregiver did not exceed 16 points when determining if an additional caregiver is needed. Evidence: 1. Upon entering the home, ten family day home children were observed in care, with one caregiver for approximately twenty-one minutes before an additional caregiver arrived. 2. Caregiver #1 reported the children in care included (1) seven month old, (1) fifteen month old, (1) seventeen month old, (1) twenty-two month old, (3) two year olds, (1) three year olds, and (2) four year olds totaling twenty-six points. 3. Caregiver #1 acknowledged an additional caregiver was needed.

Plan of Correction: Per the provider: My assistant was running late today. I will remind her to be on time and look into getting a substitute as backup.
