Comments:
A Subsidy Health and Safety inspection was conducted on April 24, 2019 to review supplemental health and safety requirements. The inspector initiated the inspection at 8:55 am and concluded the inspection at 11:05 am. Upon the inspectors arrival there were ten family day home children in care with one caregiver for a total of 26 point. Later an additional caregiver arrived. Children were observed playing with toys and looking at books at the table. Later children did the chicken dance to a music tape. Diapering and bathroom breaks and hand washing were observed. The caregivers interacted with the children in a positive manner. All areas used for child care, inside and outside, were inspected. The family day home has age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The family day home had the required information posted. The inspectors interacted with the caregivers and children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Ten children's records, two caregiver records, and one household member's record were reviewed today. Caregivers have current First aid and CPR. Medications are not administered nor does the provider offer transportation. The most recent evacuation drill was documented on 04/10/2019. The two required shelter-in-place drill were completed on 02/27/2019 and 03/19/2019. A lock down drill was documented on 02/27/2019. Today's lunch consisted of peanut butter and jelly sandwich, mashed potatoes, green beans and milk. If you have questions, please contact the licensing inspector at 804-662-9758.

Description: Based on review of children's records, the provider did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction, Evidence: The record of child #10 had documentation of a diagnosed food allergy however there was no allergy cation plan.

Plan of Correction: Per the provider: I will ask the parent to get an allergy action plan. I will print one from the web site to give her to take to the doctor.

Description: Based on review of the emergency preparedness plan, the provider did not include procedures for the continuity of operations to ensure that essential functions are maintained during an emergency.
