Comments:
An unannounced renewal inspection was conducted from 1:20pm through 2:30pm. There were five children in care including one resident child with the provider. Children in care ranged in age from 20 months to three years old. The provider failed to open the door to the inspector upon initial knock. However, after some time including a phone call to the family day home that went unanswered, the provider called the inspector back and entry was made to home. The provider explained that she and the children were engaged in an emergency evacuation drill and had exited and re-entered through the back door of the home. Children were observed in play activities. Children and staff records were reviewed as well as required postings and emergency drills. There were not any violations identified during this inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview the provider failed to ensure that each staff record contains all the required information. Evidence: 1 - The record for child 2 does not contain documentation of annual update as required. Child 2 has a documented first day of attendance as November 3, 2017. 2 - The provider confirmed that there was not an annual update documented in the record of child 2.

Plan of Correction: The provider will have the parent update the information in the record as soon as possible.

Standard #: 22VAC40-111-70-A
Description: Based on record review and interview the provider failed to ensure that before the child's first day of attendance, parents are provided in writing information regarding whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation. Evidence: 1 - The record of child 1 does not contain any information regarding the status of the family day home's liability insurance. 2 - The provider confirmed that there was not any documentation of liability insurance in the record of child 1.

Plan of Correction: The provider will provide the parent with the form and obtain the signed document from the parent.
