Technical Assistance:
Consultation provided on varying the time of day when conducting escape drills. Make sure all emergency numbers are clearly posted ( 911, Regional Poison control and Emergency Back-Up Person). Remember that the most recent findings from the last inspection must be posted so that it can be clearly seen. Please review all medication standards and until you have completed MAT you cannot accept medication into your home unless its non prescription topicals such as sunscreens etc. Consultation provided on components for lunch to include two vegetables or a vegetable and fruit.

Comments:
An unannounced renewal study was conducted today with the assistant and then the provider returned from an errand to complete the inspection. Five children were all napping at the beginning of the inspection for a total of 12 points. One child had medication on site. Six children's files were reviewed as well as two household member's files. The provider was up-to-date with the monthly escape drill and shelter in place drills. Please submit your renewal application by 5/8/19. Arrival time: 12:30 pm Departure time: 2: 20 pm

Violations:
Standard #: 22VAC40-111-980-A
Description: Based on interviews, it was determined that the provider routinely picks up or drops off the day care children at school but the provider has not obtained authorization to transport from each parent.

Plan of Correction: I will get the form signed and keep a copy in my vehicle.

Standard #: 22VAC40-111-220-A
Description: Based on interviews and an inspection of medication for one child, it was determined that the provider has prescription medication on site for one child but has not completed the Medication Administration Training (MAT) as required prior to receipts or administration of medications. Evidence: The provider has an Epipen Jr., on site for child A. The provider and her assistant do not have MAT.

Plan of Correction: I will return the medication until i get MAT.

Standard #: 22VAC40-111-500-C
Description: Based on observation, it was determined that outdoors play equipment is placed over a hard surface. Evidence: Underneath the swing set used by day care children are several bare ground spots where the grass has been worn away.

Plan of Correction: I will cover the bare ground spot with rubber mats.
