Comments:
An unannounced renewal monitoring inspection was conducted from 4:10pm to 4:55pm. At the time of inspector's arrival there were 9 children, ages 2 to 14 years, for a total of 12 points, in care with the provider. The records sample size consisted of 5 children's records and 3 provider / assistant/ household member records. Children were observed participating in departure, indoor play and educational instruction. The provider stated that currently no medication is administered to children while in her care. The provider also reported there were no incident reports since the last inspection. Areas of noncompliance are identified on the violation notice and discussed with the provider at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review an interview it was determined the provider did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #2(enrolled 1/28/19) did not contain the required written/signed emergency authorization. 2. The provider confirmed that this documentation was not available during this inspection.

Plan of Correction: The provider will request this documentation from the parent at pick up.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview it was determined the provider did not obtain a current report of Tuberculosis screening form for each caregiver and adult household member every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. The record for the provider and household member did not contain current Tuberculosis screening documentation. 2. The documentation available was dated 12/2/16. 3. The provider confirmed this was the most current documentation however; they have to go back in 48 hrs. to have their recently placed tuberculosis test read.

Plan of Correction: Provider and household member will have their TB test read and will file the documentation in the records.
