Technical Assistance:
Discussed the following with the Provider: - sample forms to be used for children's registration - point system requirements for additional caregivers - emergency plan information

Comments:
An announced initial inspection was completed today. The physical plant, 4 caregiver/household member files, caregiver training, child nutrition, emergency procedures and supplies were inspected between 2:00-3:15pm. There are no children enrolled to receive care at this time. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Once the results of the search of the central registry are reviewed by the Licensing Inspector, a conditional license will be issued. Description: Based on observations, the first aid kit did not include all required items. Evidence 1. The First aid kit is missing scissors, tweezers, a digital thermometer and triangular bandages.

Description: Based on interview, the Provider does not have a working battery-operated flashlight, a working portable battery-operated weather band radio, and extra batteries. Evidence 1. The Provider informed the LI that she does not have these items.

Description: Based on observations, the Provider does not have all required posted telephone numbers Evidence 1. A 911 or local dial number for police, fire, and emergency medical responders number is not posted. 2. The regional poison control center number is not posted.

Description: Based on file review, the Provider does not have a written emergency preparedness and response plan. Evidence 1. The Provider has a blank Emergency Preparedness plan that needs to be completed.
