Comments:
An unannounced renewal inspection was conducted today between 12:40pm and 2:40pm. There were two children (7 points) with one caregiver. The entire physical plant was inspected. Caregiver/household members/children's records, emergency supplies, cribs and evacuation logs were reviewed. The children were observed playing and interacting with caregiver. Standard #: 22VAC40-111-180-A
Description: Based on household record review, the provider failed to obtain for each adult household member a current Report of Tuberculosis Screening form in accordance with requirements. Evidence: Household member #3 does not have current Report of Tuberculosis Screening.

Plan of Correction: I will have household member #3 obtain TB test and document results in file.

Standard #: 63.2-1721.1-B-2
Description: Based on review of household records, provider failed to ensure that household members 18 years old or older submit to fingerprinting and provider personal descriptive information. Evidence: Household member #3 does not have documentation of fingerprinting record in file.

Plan of Correction: I will have household member #3 submit to fingerprinting.
