An unannounced mandated Monitoring inspection was conducted today. There were 2 children in care (6 points) with 1 caregiver. Children were observed reading books, having their diaper changes, hand-washing practices and napping. Reviewed 2 children's files, 1 caregiver file, 1 resident file, attendance, emergency evacuation log, cribs and physical plant. Violations of the Standards are recorded on the Violation Notice. Evidence: Child#1 was missing parent employment address and the name, address and telephone number of one designated person to contact in case of an emergency if the parent cannot be reached Plan of Correction: I will get that information from the parents.
